The ‘Pathways’ Leaving Care team in Middlesbrough is seeking to recruit a Leaving Care Personal Advisor to support our young people in their transition into adulthood.
Pathways provides quality services to young people in accordance with the children (Leaving Care Act) 2000 and the Children Act 1989, guidance and regulations volume 2 and 3.
The roles and responsibilities will include:
Working with young people who are Looked After and Care Leavers, aged 16 to 25 years.
Working with unaccompanied Asylum Seekers.
Providing a holistic service to young people, assessing needs to provide the best possible support, advice, guidance, and planning.
To work in partnership with key agencies and support networks.
To complete Pathway Plans that are robust, reflective, and based upon individual needs.
To support young people in making wise choices, being safe and reaching their full potential.
Supporting the housing needs of our young people making their transition into adulthood.
Supporting our young people in meeting their educational potential and engagement in education, employment, and training.
Supporting our young people to meet their health needs.
To ensure or young people wishes, and feeling are at the centre of all practice and that their views help to shape interventions and practice. Which is recorded within their Pathway Plan.
To ensure that accurate records are maintained which reflets decision making.
Support the team and young people through the duty service and social groups.
The successful candidate will have a genuine interest in working with young people and have an enthusiastic and dynamic approach to their work.
